Maybe it was Jacob's place at one time and he was able to escape to the foot once the ring was disturbed? But, wouldn't Richard and the others never have allowed the ring to imprison him in the first place? So much to think about, lol.

For some reason I got the impression that the ring was meant to keep Smokey/Anti-Jacob out. Once it was broken that is why Jacob left? And maybe Not Jacob moved in?

I expected Jacob to be at Sarah's surgery too. It would have been funny though if Jacob saw Jack as a kid and made him promise not to get a tatoo in Thailand. I'll be extremely disappointed if Jacob is dead. I want him to be alive and save Locke.
