I've had a trip planned for the last six months, and it is well known through various channels that I will NOT be around during the month of August. The wedding as you may have guessed, is in August. So inside the envelope is another envelope where apparently I am supposed to check off a form stating whether or not I will be attending. These fuckers just want money don't they? I haven't talked to this fucking person in over a decade. Will the family ostracize me if I don't give? What the FUCK do I do?

Check no, scribble in a PS that you've got a long-planned trip and wish the happy couple well. Do it sooner rather than later. It's more likely that your aunt, being sister to one of your parents, simply wanted to make sure all of the immediate family is invited, and she might actually be genuine in wanting you there. Weddings have a way of reestablishing intra family relationships that fizzled out. Do not send a gift with the return envelope! Wait until after the wedding and either send an Amazon gift certificate or prior to the wedding take the trouble to find where they're registered and pony up for a gift in your price range. The bride's your first cousin. That's definitely not money-hogging territory. Why the hostility? If there hasn't been a family breach, play nice.

Usually I just say NO and return but if it's family or someone I know rather than some old friend or distant whatever..I say NO and send a gift.

They sent you an invitation as a courtesy since you are family. You're never obligated to send a gift, but it's considered a courtesy to send the RSVP so they can plan on the number of people attending. If they hold it against you that you don't send a gift, it's their problem, not yours.
