And the reveal is usually Winner/Mole/Loser because that's what makes the most sense suspense-wise:
Winner/Loser/Mole would take away from The Mole's reveal.
Mole/Winner/Loser would take away from the winner's reveal since "Who is the winner?" is always secondary to "Who is The Mole?"
Mole/Loser/Winner would be a total mess.
Loser/Winner/Mole has been done before (Season 1) to great effect but ultimately takes away from The Mole's reveal.
Loser/Mole/Winner would take away from the winner's reveal.
